COVID-19 Policy Violation Reporting Form

Please use this form to submit concerns for any student, faculty, or staff members in the Goucher community who may not follow Goucher College COVID-19 policies. This form may be submitted anonymously. Please provide enough information for College staff to follow up with your concern. Sometimes the ability to address the situation may be limited if staff have no one to contact about the initial report.  Use this form to report COVID-19 health concerns.  

Faculty and staff concerns are sent to the Office of Human Resources, and concerns for students are sent to the Office of Vice President of Student Affairs & Dean of Students and handled through the Student Conduct process.
