Disclosing & Documenting a Disability

Incoming students with documented disabilities who wish to request accommodations must complete the Accommodation Request Form below and send it to the Office of Accessibility Services.

To ensure the college is able to meet the needs of students with disabilities in a timely manner, documentation and diagnosis of a specific disability completed by a qualified examiner should be mailed, emailed or faxed (410-337-6185) to the Office of Accessibility Services as soon as possible.  If submitting information for a new academic year, we request that you submit your requests by June 15 preceding the fall semester.

We request that you complete the Accommodation Request Form online, if possible. Alternatively you can complete a hard copy of the Accommodation Request Form (PDF) and send to:

After your request is submitted, OAS will contact you to make an appointment to discuss your requested accommodations. You will be asked to provide a treating provider form from your healthcare provider supporting your request for an accommodation. Ask your provider to complete the form below and send to OAS at the above address or bring the documentation with you to your appointment.

If your request for an accommodation is denied, you may appeal the decision. Please refer to the college's Disability Accommodation Appeal Policy (PDF).

Confidentiality Statement

Disclosures to OAS are confidential and will never appear on a student's college record. OAS does not share information regarding eligibility for accommodations with other college staff or faculty members unless authorized in writing by the student. Students are welcome to disclose a disability and receive guidance without needing to request or use any accommodations.
