Help Desk Request Form

The Help Desk provides a single point of contact for all computer, telephone, and voice mail questions, problems, information, or service requests for faculty, staff, and students. This request form provides the campus community with a way to submit questions, problems, or requests to the Help Desk through the web. Your assistance in providing the detailed information below will assist the Help Desk in understanding your request and aid the Help Desk in contacting you in a timely fashion.
