Account Information Release Form

The Account Information Release Form is on the back of the OneCard brochure. Each student will be given this brochure when they receive their new OneCards. The forms can be removed from the brochure and given to the OneCard office on Opening Day or can be dropped off at the Controller's Office in Dorsey Center where the OneCard office is located.
