Health Insurance Waiver/Enrollment Instructions:

Go to www.hulseqm.com/myinsurance and follow the instructions to Enroll/Waive.

Use the following information to log in:

Login ID:

The last 5 digits of your Goucher Student ID located on the top right corner of the Goucher Statement of Account.

Password:

The student's last name first initial entered without spaces.  Enter John Smith as smithj

No response by September 30, 2009 will result in an automatic  $850 charge to the Student Billing Account in October.
