Medical Leave of Absence During the Semester

• Students having an emergency during the semester must obtain the Leave of Absence form from the Student Administrative Services, the dean of student's office or the associate dean of undergraduate studies office.
• The student must make an appointment with the dean of students, associate dean of students or the associate dean of undergraduate studies.
• Students must submit Leave of Absence form to Student Administrative Services for financial aid and student billing to sign.
• Faculty will be notified by the registrar's office and the student will receive a "W" in all courses.
