Students who have a medical or other type of personal emergency during the semester and need to withdraw from their courses must make an appointment with the Assistant Dean for Student Development (ADSD).
After the add/drop period has ended and before week 10 students can withdraw and receive a "W" in their courses. The official date of withdrawal will be the last date of academic activity as determined by the college.

This form is used when a student wishes to withdraw from all classes during the semester. The form should also be used for voluntary medical or compassionate withdrawals.
