Required Signatures on Registration Forms

All course change forms require the adviser's signature.

Adviser's and instructor's signatures are required for:

• Courses that are added once classes begin.
• Courses that are closed.
• Audits
• Pilates (Dan 102)
• Time conflicts require the signature of the instructor for the course you will be missing time in, and the signature of the instructor for the course you will be attending during this time.

Adviser's and music secretary's signatures are required for:

• Private music lessons

Adviser's, instructor's, and department chair's signatures are required for:

• Independent work
• Courses pursued independently
